Madison PTA Mini Grant Request Form for Teachers

Madison PTA has allocated funds for a Mini Grant Program to provide teachers and staff with money to support projects which are not funded through other sources. Requests could include such things as books, musical instruments, PE equipment, computer software, cameras, or science equipment. Please submit the following Mini Grant Request form to Angela Puckett, PTA President.
