Madison School 2007-2008 Yearbook

The Madison Elementary School 2007-2008 Yearbook is a great way to remember your child’s year at school.  Work on the next edition of the yearbook begins NOW and we want YOU to be a part of it!

Photo Release Forms

In order for us to be able to include your child's picture in the yearbook, we must have a completed photo release form on file for the child, indicating the parent or guardian's approval to include their name and picture.  Look out for a copy of the photo release form in your Back-To-School packet, which will be given to your child on their first day of school, and return it to the School Office as soon as possible.  You can also find a copy of the form to print out at home on our Downloads page.

If we have not received your photo release form by 1st March 2008, we cannot guarantee that your child will be included in the yearbook.

If you do NOT want your child's picture to appear in the yearbook, please indicate this on the form.  We can either print your child's name but no picture, or we can omit the name and picture altogether. 

Send Us Your Photos!

We need lots of candid photos of students, teachers and parents! If you take any pictures this year of your child and their friends in class, on field trips, at school concerts, events, evening functions or any other aspect of school life, please remember to share them with us. We prefer high-resolution digital photos but can also accept color prints.

Design Our Clip Art!

Students—do you like to draw? We need “clip art” to decorate the yearbook pages and we’d like YOU to design it! We need designs along the following themes: Academic, School Supplies, Art, Patterns, Symbols, Weather, Seasons, Halloween, Christmas, Hanukkah, Space, Nature, People, Animals or you can draw anything else that you want. Designs should be in black and white ONLY and at least 3 inches square. Please make your design nice and bold (e.g. use a thick black marker pen) because it may need to be reduced in size for printing.

Help Wanted!

Would you like to design one of the pages for the yearbook?  Or are you available to help collect orders and write receipts?  We would like to hear from you!  Help is needed until the end of March, and then on day later in the year where yearbooks are delivered.

Submission Deadlines for Photos and Art

Please place your photos and artwork in an envelope marked “Yearbook Committee” and either give them to your child’s teacher or hand them in to the School Office by March 31st, 2008. Please mark your child’s name and room number on the back of each item and mark clearly if you want any items returned.

How to Order Your Yearbook

Look out for order forms coming home now in your child's Thursday packet!

The Madison Elementary School 2007-2008 Yearbook is a great way to remember your child’s year at school. Remember to order copies for yourself, your child/ren and your relatives too!

This year’s book includes:

• 60 pages in FULL COLOR!
• Tons of photos of this year’s school events and activities
• Dedications for 5th grade students (see separate flyer—5th grade ONLY)

Price per yearbook: EARLY BIRD SPECIAL: $16, (UNTIL FRIDAY FEBRUARY 29TH) or $20 if ordered after Friday February 29th

ORDERS MUST BE RECEIVED BY MONDAY MARCH 31ST TO GUARANTEE DELIVERY!!

We will issue a receipt to acknowledge your order. If you do not receive it within a week of placing your order, please contact

Cash, check or money order only. Please make checks and money orders payable to “Madison PTA” and write “Yearbook” in the memo section. You may use one order form per family if you have more than one child at school.

Please place this form, along with your payment, in the attached yearbook order envelope and return it to your child’s teacher or the school office as soon as possible . We regret that we CANNOT accept any orders placed after Monday March 31st. Thank you for your order!

Yearbook Delivery

Pre-ordered yearbooks will be distributed to students after school on Wednesday June 11th, 2008. Pickup times are as follows:

• Kindergarten: 11:15-11:45 a.m. by the kindergarten gate
• All grades: 1:30-3:00 p.m. on the main playground

All uncollected yearbooks will be delivered to your child's classroom. If you need to make special arrangements to collect your yearbook, please contact Yearbook Coordinator Jackie Romano at bondiofab@verizon.net.

NOTE: If you did NOT pre-order a yearbook, there are a few spare copies available for sale at $20 each. These can be purchased on a first-paid, first-served basis. If you would like to buy a copy, please return the order form in your child's Thursday packet by Tuesday June 10th or bring $20 cash to school on Wednesday.
